Is a Chiropractic Neck Adjustment Safe? What a 2026 Study of 860,000+ Adults Found
If you’ve ever felt nervous about having your neck adjusted, you’re not alone. One of the most common questions we hear at Above Down Chiropractic is whether a
neck adjustment can cause a stroke or damage an artery. A large new study gives a clear, reassuring answer.

The question
A cervical artery dissection (CeAD) is a rare tear in one of the arteries in the neck. In some cases it can lead to a stroke. For years, people have asked whether chiropractic neck adjustments might trigger this kind of injury.
The tricky part is that a dissection often starts with neck pain or a headache. Those are exactly the symptoms that bring people to a chiropractor. So when a dissection is found after a visit, it can be hard to tell whether the adjustment caused it or whether it was already happening.
What the researchers did
A team of researchers from University Hospitals Cleveland, Duke University, Cleveland Clinic, and other institutions published this study in 2026 in the journal Chiropractic & Manual Therapies. The team included specialists in orthopedics, neurosurgery, cardiology, physical therapy, biostatistics, and chiropractic.
They reviewed U.S. health records and insurance claims from 2010 to 2025 and compared two groups of adults who sought care for new neck pain or headache:
About 431,000 adults who received a chiropractic neck adjustment
About 431,000 similar adults who saw a medical provider and were prescribed ibuprofen
The groups were carefully matched on age, sex, health history, migraines, blood pressure, smoking, and dozens of other factors. The researchers then tracked who was diagnosed with a cervical artery dissection over the next 30 days.
What they found
Dissections were very rare in both groups, roughly 7 to 13 cases per 100,000 people.
Chiropractic patients did not have a higher risk of cervical artery dissection than patients treated with ibuprofen.
Dissections showed up at a similar pace in both groups after people first sought care, no matter which treatment they received.
That last finding matters. It supports the idea that neck pain or headache can be an early warning sign of a dissection already in progress, rather than a result of the adjustment.
What this means for you
This is one of the largest studies ever done on this question. It adds to a growing body of evidence that neck adjustments by a chiropractor are not linked to an increased risk of cervical artery dissection.
The researchers also emphasized that every provider, including medical doctors and chiropractors, should stay alert for warning signs. That’s why every new patient at Above Down starts with a thorough history and exam before any adjustment. If something in your history or symptoms points to a concern outside our scope, we refer you to the right provider.
Warning signs to know
Seek emergency care right away if you have neck pain or headache together with any of these:
A sudden, severe headache unlike any you’ve had before
Dizziness, vision changes, or trouble speaking
Numbness or weakness in the face, arm, or leg
Trouble walking or loss of balance
Frequently asked questions
Can a chiropractic adjustment cause a stroke? This 2026 study found no increased risk of cervical artery dissection, a leading cause of stroke in younger adults, after chiropractic neck adjustments compared with standard medical care.
Why do some people link chiropractic care to strokes? Dissections often begin with neck pain or headache, which leads people to seek care. That timing can make it look like the visit caused the problem when the dissection was already developing.
Is chiropractic care safe for neck pain and headaches? Large studies, including this one, support chiropractic care as a safe option for adults with neck pain and headaches when it is provided by a licensed chiropractor after a proper evaluation.
